Risks and aftercare information
Before the procedure, I was informed that a tattoo is permanent, that it may be painful, and of the possible risks: infection, allergic reaction to pigments, abnormal healing (keloid scarring) and changes in colour over time.
I was informed of the aftercare to follow: keep the dressing on for the time indicated, wash with a mild soap, apply the recommended aftercare product, do not scratch, avoid baths, swimming pools, the sea, saunas and sun exposure while it heals (about 3 to 4 weeks), and see a doctor if redness, heat or pain persists.
Removing a tattoo requires laser sessions that are long and costly, and the result is not guaranteed.

Tattoo consent form FAQ
Do I need a tattoo consent form?
In most countries, artists are expected to inform clients about the risks and aftercare before the procedure, and many local health authorities require written consent. Even where it is not mandatory, a signed form is the simplest way to show that the client was properly informed.
Can I tattoo a minor?
It depends entirely on where you work: some places ban tattooing minors, others allow it with written parental consent. Only use the parental consent section where local law permits it, and check the exact conditions with your local authority.
How long should I keep signed consent forms?
Retention periods vary by country. Keeping them for several years helps you show what information was given if a dispute arises. Store them securely and only collect the data you need, in line with the privacy law that applies to you.
What should a tattoo consent form include?
The client's details, a health questionnaire, information on the permanent nature of the procedure, its risks and aftercare, parental consent where applicable, a photo release and the client's dated signature.
Does it work for permanent makeup and piercing?
Yes. Pick the type of procedure in the tool: the health questions and the risk and aftercare information adapt to tattooing, permanent makeup or piercing.
